Combined Charcot hindfoot and midfoot reconstruction using internal fixation method—surgical technique and single surgeon series

: Isolated midfoot and hindfoot Charcot reconstruction using internal fixation is increasingly a common procedure in multidisciplinary diabetic foot units, the surgical techniques have well been described. However, about third of deformities that require limb salvage present with involvement hindfoot. Surgical combined deformity an evolving technique technically challenging. We correction stabilisation fixation, developed by senior author (VK), outcomes. All patients had undergone to address threatening due neuroarthropathy, performed author, minimum follow-up 12 months, included this study. The principles adequate pre-operative optimisation patient, sequential followed principle long-segment rigid optimal bone opposition. Standard post-operative regime, including offloading, has used all patients. A total 34 (35 feet) between January 2009 December 2019. Active ulcers were noted 13 feet at time procedure. Eleven reconstructions as two-stage procedures presence active infection. At mean 53 11/13 ulcer healed, 32 (33 full weightbearing shoes or brace latest follow-up. Bone fusion was 28 region midfoot. Metal work failure 5 requiring removal 3 feet. Revision required 4 Our newly described provided functional majority presentations, acceptable level complications, medium-term months.
